Referral Coordinator

Job Summary
The Referral Coordinator is responsible for facilitating and tracking all patient referrals to outside service providers. Serves as the liaison between Primary Health Care, Inc. and community service providers. Coordinates with PHC team members and providers to ensure a positive experience for all patients.
Essential Functions
- Coordinates all referrals made from Primary Health Care, Inc. locations to outside service providers on a daily basis, with the exception of those handled by clinical staff and in-house referrals.
- Ensures that high priority referrals are processed in a timely manner in accordance with workflows.
- Ensures appointments are scheduled and kept by communicating with patients and service providers.
- Monitors referral tracking daily to ensure referrals have been coordinated properly.
- Identifies any barriers that would prohibit or delay patients from receiving necessary care.
- Tracks details of why appointments were not kept to inform Continuous Quality Improvement initiatives and guide dialog with third party service providers.
- Serves as a liaison amongst patients, providers, nurses, and patterning agencies.
Minimum Qualifications
Education: High School Diploma or equivalent.
Experience: One year relevant work experience in medical field.
License/Certifications: None required.

- Ability to utilize Electronic Medical Records (EMR)
Other Job Requirements: Must be at least 18 years of age.
Preferred Qualifications
Education: Medical terminology course work.
Experience:
- Experience in referral management.
- Work experience in a community health center.
License/Certifications: CMA or LPN
Knowledge, Skills & Abilities: Bilingual, verbal and written language proficiency; English/Spanish.
Typical Work Hours
This position typically works 40 hours per week.
Compensation & Benefits
Compensation is dependent on education and experience. PHC offers a comprehensive benefits package, including:
- $2,000 sign-on bonus ($1000 at hire, $500 at 6 months, $500 at 1 year) **Internal Applicants are not eligible for the sign-on bonus**
- Qualified individuals are eligible for up to a $500 quarterly retention bonus as part of special funding through the American Rescue Plan Act (ARPA). Bonuses are time limited through the Spring of 2023.
