Case Manager with The Project at Primary Health Care

Job Summary for the Project

The Case Manager I is responsible for assisting clients being served in one of PHC’s social service programs (people experiencing homelessness or those who are living with HIV/AIDS) through needs assessment, case management, and advocacy with community partners. The case manager provides services according to grant guidelines. Contact may occur in non-traditional settings. 

Essential Functions

  1. Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services. May assist clients with application process for entitlement programs.
  2. Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.

  4. Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies, including emergency and crisis services. 
  5. Advocates for clients with community agencies, schools, significant others and other appropriate agencies. 
  6. As appropriate, ensures that clients have access to transportation to necessary appointments, and assists with client transportation according to the PHC transportation policy. 
  7. Uses professional discretion with client information while communicating with other agencies on behalf of clients, ensuring that appropriate releases of information are obtained. 
  8. Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as a liaison between PHC and community resources. 

 

Minimum Qualifications

Education:   Bachelor’s degree in psychology, counseling, mental health, human services, or related field. Four years of experience in essential functions of position may be substituted for advanced education if allowed by granting agency.

 Experience:   None required for applicants with a Bachelor’s degree.

 License/Certifications: 

  1. *Active Chauffer’s license from Iowa Department of Transportation. 
  2. *Current Mandatory Reporter Training: Child and Dependent Adult Abuse recognized by the Iowa Department of Public Health.

 * Must be obtained within introductory period if not current. 

 Knowledge, Skills or Abilities:

  1. Excellent customer service skills.
  2. Effective English communication skills, written and verbal. 
  3. Contributes to a team approach to care.  
  4. Proficiency with Microsoft Word & Excel and the ability to utilize database applications. 
  5. Demonstrates attention to detail in work.  
  6. Prioritizes assigned work. 
  7. Establishes and maintains effective working relationships with a diverse group of individuals.
  8. Interacts with others in an empathetic manner while maintaining professional boundaries. 
  9. Remains calm and works through difficult interactions. 

 Other Job Requirements: 

  1. Must be at least 18 years of age.

Education: N/A

Experience:

  1. Six months of work or volunteer experience in human services related activities.  (Will consider experience acquired while obtaining the degree.)
  2. Work experience with vulnerable populations, including people experiencing homelessness. 

License/Certifications: N/A

Knowledge, Skills & Abilities: 

  1. Demonstrated competency using HMIS or CareWare as applicable to department.
  2. Working knowledge of community resources related to homelessness or HIV/AIDS in Des Moines.
  3. Knowledge of program terminology.
  4. Ability to effectively use HMIS or CareWare system to document all client contact.
  5. Demonstrate language proficiency in Spanish. 

 

Typical Work Hours

This position typically works 40 hours per week.

 

Compensation & Benefits

Compensation is dependent on education and experience.
