Nurse (Clinical Care Coordinator)

Salary: $78,659 to $111,630 per year

Summary

ORGANIZATIONAL LOCATION: Department of Health & Health Human Services (HHS), Indian Health Service (IHS), Southwest Region, Phoenix Area -Nursing Administrative Services, Colorado River Service Unit, Parker Indian Health Center. Position is located in Parker, Arizona and Supervised thru Supervisory Nurse for Outpatient Nursing Services. Primary function is to ensure patients receive quality patient within the outpatient, urgent care, specially care and medical home care teams.

Responsibilities

Major Duties:

  • Implements and initiates standardized disease management and health prevention guidelines across the patient care continuum. Assess patient's needs individually and/or as a group with the care team. Collects subjective and objective health status data from multiple sources, which includes the patient, family members, team members and documented records. Assists with triage of patients, including triaging telephone calls and walk-in patients. Assesses patient needs, asks appropriate questions, and uses clinical judgment, knowledge and experience in directing patients. Utilizes standards of care when providing self-care advice, referring to primary care provider or specialist, making an appointment or instructing the patient to seek emergency medical 911 assistance.
  • Develops a plan of care with the care team according to clinical practice guidelines with the patient, family, caregiver(s) and significant other(s). Identifies resources to integrate additional clinical practice guidelines to support continuity of care.
  • Evaluates the outcomes of patient care and modifies the plan of care based on outcomes, individually or with the team. Evaluation includes documentation of the reassessment of patient status, evidence of patient and family participation and responses, and the rationale for the revisions in the plan of care. Participates in designing improved clinical care delivery systems and identifies opportunities and set priorities to continuously improve nursing and organizational performance through data collection and monitoring activities set by nursing, the care team and the Service Unit. Initiates therapeutic interventions and administers medications as prescribed by medical providers.
  • Lead Duties: Manages and works with clinical and non-clinical health care staff to ensure improvements in access to care. Proactively develops relationships with clinical and non-clinical health care staff. Communicates efficiently with clinical, non-clinical health care staff, and patients.

BASIC REQUIREMENT(S):

Education:

A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing EducationCouncil on Accreditation of Nurse Anesthesia Educational ProgramsAccreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

Licensure:

For all grade levels and positions, applicants must have passed the National Council Licensure Examination In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below

MINIMUM QUALIFICATIONS- GS-610-10:
  • (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Develops and maintains patient registries based on disease, health condition and/or health prevention initiatives. Ensures complete pre-visit planning for patients and coordinates referrals and outside services received or needed by patients.

MINIMUM QUALIFICATIONS- GS-0610-11:
  • (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Care management involving patient assessment, initiating, and maintaining individualized multidisciplinary plan of care involving patients and their families to give optimal care. And/or
  • Completed a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field.

Foreign Education: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show that the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
